DROP OFF INFO FOR OAKBANK RCMP DETACHMENT:
Remember to bring two 2 pieces of identification to the Oakbank Detachment (e.g. drivers license, health card) when submitting your application.
Oakbank RCMP Detachent is open M-F 8am-4pm.  They are closed betwen 1pm-2pm for lunch.

2a.2023/2024 Season Child Abuse Registry Check Application Form
Please print and complete application form.  If you do not have a printer email completed forms to secretary@icehawkshockey.ca prior to Oct.11/23.

• SMHA will submit any applications handed in at the coaches meeting (Oct.11, 7-9 pm Oakbank Arena Board Room) or emailed in prior to Oct. 11th.
• If your application is not submitted prior to Oct.11th you will be responsible for submitting the paper work on your own to 777 Portage Ave, Winnipeg. 
• Processing time is appoximately 2-3 weeks. 
• All clear checks have a due date of Nov. 15/2023 for bench staff to submit to the team coach/manager.

(For Coach/Manager only)

Bench staff:

• Once you receive your clear checks please show and submit a copy to the Coach/Manager of each team

Coach/Manager

• Please use the Declaration form to keep track of all bench staff who have completed their checks. 
• Submit completed declaration form to secretary@icehawkshockey.ca by Nov.15/23. 
• If a bench staff has not completed these forms by Nov.15/23  it is up to the coach/manager to notify SMHA Executive.
